1. Coordinating the delivery of health check session in community venues, and liaising with the health checks delivery team based at Northenden Medical Practice.
2. Work with the wellbeing service on provision of lifestyle advice and community knowledge.
3. Researching venues in Manchester and identifying suitable venues to ensure constant availability of venues. Use community knowledge of buzz workers to identify suitable venues, and ensure venues are suitable and accessible
4. Coordinating an appointment booking system, so that sessions are booked 6 months in advance where possible, and that GP practices send out invitations when requested.
5. Be the face of the community health check programme and promote the service to all stakeholders.
6. Promoting and encouraging potential patients to undertake a health check by being proactive and discussing the benefits of having a health check.
7. If required train to deliver health checks in community settings, such as finger prick sampling, BP readings, body measurements, and very brief lifestyle advice.
8. Encouraging patients to commit to lifestyle changes and accept referrals to wellbeing services. Work with other stakeholders who have a role to play in communities and in improving health outcomes.
9. Engaging with GP practices and GP federations to promote health check uptake, and refer patients as necessary to GP.

Pay: ₹15,000.00 - ₹25,000.00 per month
